For example, newborn reflux is a common occurrence and can only be avoided when a proper newborn nursing diagnosis is done. It only requires the skills of a nurse to take care of the baby and the diagnosis is fairly easy if done with the right procedure in mind. This helps preserve the baby’s health and it also helps guide parents to take care of the baby. For a newborn nursing diagnosis you have to do the following steps. Being gentle throughout the process is key, especially when the baby is sleeping so you can get the best assessment possible.
The first thing you need to do is to sanitize your hands, because the baby’s skin and condition is very sensitive. If the baby is sleeping then it is the best time to assess the baby’s respiratory rate. This is very easy, you just have to lay your hand on the baby lightly and you can count the respiratory rate while watching at the clock. You should do this for a full 60 seconds until you get the full idea of the baby’s respiratory rate. Second thing you need to see if the heart rate of the baby. This needs to be done delicately because waking up the baby could upset the heart rate and will disturb your assessment.
Take a clean stethoscope and place it on the baby’s chest to measure the heart rate. Count the heart rate for a full minute and don’t be surprised by the phenomenal heart rate. This is normal for newborns and if you find a count of over 120 or 160 then that is a normal sign for a newborn’s heart rate. Once you have these two figures ready and the signs are good, you need to look in to the breath sounds. You can do this by places the stethoscope on the skin of the baby at different places and inspect the sounds. Next, take a thermometer and look for the fontanel of the baby. Use your figure to lightly notice the suture line of the baby’s head. This way you can tell if the sutures are flat, overriding or separated. If the baby is awake and alert, that is fine because you have done the delicate measurements.
Next thing you are going to check is the ancillary temperature of the baby. You can un-wrap the baby and place the thermometer to get the readings. Make sure you do this tight until you hear the beat of the baby. Now that you have taken the temperature, you need to listen to bowel sounds. At this point you can take the baby on the side and notice the breath sounds on the back. Make sure you are holding the baby delicately with the head. The neck is very weak at this time which is why you need to take care of the head.
Next you can see the hand grasps and as a reflex they will hold your hand tight and that is perfectly normal. Now the baby is ready and you just need to change the diaper. Now just observe the genitalia are normal making sure there are to rashes or so. If there is a stool, then make a note of that because that is a strong indicator of the baby’s health. Finally observe symmetry of the baby and check the limbs if they are moving in normal direction. Finally put fresh t-shirt on the baby and hold the baby upright and wrap the baby with a fresh blanket back on the bed.
